Equalizing Your Ears Early and Often

The single most effective technique to avoid ear barotrauma is to master and consistently perform ear equalization. The pressure change you feel is due to water pressure compressing the air space in your middle ear. To counteract this, you need to gently push air from your throat into your Eustachian tubes, which connect the back of your throat to your middle ear. The key is to start before you feel any discomfort. A good rule is to equalize every two feet of descent, starting the moment your head goes below the surface. Don't wait for pain; that's a sign you've already let the pressure differential build up too much. The most common and gentle method is the Valsalva maneuver: pinch your nose shut through your mask skirt and gently blow out against your pinched nostrils. You should feel a "pop" in both ears. It must be a gentle blow; forcing it can damage the delicate structures of the inner ear.

Controlling Your Descent Rate is Non-Negotiable

Rushing your descent is a primary cause of ear injuries. A rapid descent dramatically increases the pressure on your eardrums faster than you can safely equalize. The general guideline for a safe descent rate is 30 feet (9 meters) per minute. This is slower than many new divers realize. As you can see, the most significant relative pressure change happens in the first 33 feet of water, which is exactly where many divers, especially those using a compact portable scuba tank, feel the urge to descend quickly to "save air." This is a dangerous miscalculation. Use a descent line if available, or watch your dive computer's descent rate indicator like a hawk. If you feel any pressure building, stop your descent immediately. You can even ascend a foot or two to relieve the pressure, equalize successfully, and then continue down slowly.

Pre-Dive Preparation and Health Checks

What you do before you even get in the water plays a massive role in preventing barotrauma. The health of your Eustachian tubes is paramount. If they are swollen or blocked by mucus from a cold, allergies, or even dehydration, they will not open properly, making equalization difficult or impossible.

Avoid diving if you have any congestion. A simple test is to try equalizing on land. If you can't easily "pop" your ears while on the surface, you will not be able to do it underwater. Stay well-hydrated in the days leading up to your dive, as this helps keep mucus thin. Some divers find that avoiding dairy products 24 hours before a dive can reduce mucus production. If you have chronic allergies, consult a doctor about safe decongestants, but be warned: diving with medication that wears off mid-dive can cause a "reverse block" on your ascent, where expanding air becomes trapped. Never use medicated nasal sprays immediately before a dive for this reason.

Advanced Equalization Techniques

If the Valsalva maneuver doesn't work well for you, there are other, often more effective, techniques that engage the muscles that open the Eustachian tubes more naturally.

  • Frenzel Maneuver: This is popular among technical divers and freedivers. Instead of using your diaphragm to blow air (like in Valsalva), you close your vocal cords and use the back of your tongue as a piston to compress air in your throat, forcing it into the Eustachian tubes. It's more controlled and puts less strain on the ear.
  • Toynbee Maneuver: Pinch your nose and swallow. Swallowing naturally opens the Eustachian tubes, and pinching your nose increases the pressure in your throat slightly. This can be very effective for minor pressure adjustments.
  • Lowry Technique: A combination of Valsalva and Toynbee; pinch your nose and gently blow while swallowing.
  • Voluntary Tubal Opening: With practice, some divers can learn to tense the muscles in the back of their throat (the soft palate) to open their Eustachian tubes without using their hands or nose at all. This is the holy grail of equalization for many.

Practicing these techniques on dry land can help you find which one works most reliably for your anatomy.

What to Do If You Experience Trouble

Despite your best efforts, you might still feel pressure building up. Your action plan is critical. First, stop your descent immediately. Signal to your buddy that you have an ear problem. Next, try to equalize again. If that doesn't work, ascend a few feet until the pressure feeling subsides. The reduced pressure will often allow you to equalize successfully. Once equalized, you can attempt to descend again, even more slowly than before. If you still cannot equalize after a couple of attempts, the dive must be aborted. Ascend to the surface, exit the water, and call the dive for the day. Forcing a descent with unequalized ears will almost certainly result in a ruptured eardrum, which is excruciatingly painful, can cause vertigo, and exposes the middle ear to infection, requiring medical attention and keeping you out of the water for weeks.

Post-Dive Care and Recognizing Injury

After the dive, pay attention to how your ears feel. A slight feeling of fullness is normal, but it should clear quickly, often with a pop when you swallow. Signs of barotrauma include persistent pain, a feeling of liquid in the ear (which could be blood or serous fluid), muffled hearing, or tinnitus (ringing in the ears). If you experience any of these symptoms, you should seek evaluation from a doctor, preferably one with experience in diving medicine. They can examine your eardrum to assess the damage. Minor barotrauma might just require rest and decongestants, while a perforation will need more careful management. Documenting the incident is also helpful for understanding your personal equalization challenges for future dives.
